Pursuant to Article 13 (1) and (2) of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (Official Journal of the EU L 119 of 4 May 2016, p. 1, as amended) (hereinafter: the GDPR), please be advised that:
1. The controller of your personal data is the Henryk Niewodniczański Institute of Nuclear Physics of the Polish Academy of Sciences, ul. Radzikowskiego 152, 31-342 Kraków,
2. The Data Protection Officer can be contacted at dpo@ifj.edu.pl,
3. Your personal data will be processed on the following grounds:
a) Art. 6 (1) (b), (c) of the GDPRin order to properly organize and settle the conference,
b) Art. 6 (1) (a) of the GDPR in order to inform about future scientific events organized by IFJ PAN and publish lists of event participants and conference photo on websites related to the activities of IFJ PAN.
c) Art. 6 (1) (a) of the GDPR in order to prepare meals during the workshop dinner in accordance with your food preferences,
d) Art. 6 (1) (a) of the GDPR in order to organize online version of the workshop (concerns your image and voice sound).
4. Your personal data may be disclosed to the following recipients or categories of recipients:
a) entities that cooperate with IFJ PAN as part of scientific cooperation and providers of necessary services under the relevant agreements (e.g. courier, postal services),
b) entities and institutions authorized to access or obtain personal data under provisions of law (e.g. the police, court),
c) hotel in order to make a reservation,
d) anyone who has access to the Internet and the lists of conference participants and photos published there – if you consent to the processing of your personal data for the purpose of publishing the list of conference participants on the website.
e) Zoom instant messaging provider.
5. Your personal data will be retained:
- under the provisions of the law on archiving documents and in accordance with the Uniform Material List of Files in force at IFJ PAN for a period of 5 years, counting from 1 January of the year following the final settlement of the case (data necessary for the purpose of the organization and accounting of the conference),
- in order to inform you about future scientific events organized by IFJ PAN until you have withdrawn your consent to the processing,
- in order to publish the list of participants and conference photo on the conference website, to prepare meals in accordance with your food preferences and to use Zoom instant messaging - until the end of the conference.
- until the end of the conference. Moreover, by participating in the hybrid meeting workshop you agree to have your talk recorded and stored for future access by the CREDO collaboration.
6. Your personal data will not be subject to automated processing, including profiling.
7. You have the right to demand the exercise of your rights under the GDPR, i.e.:
- the right to access your data and the right to receive a copy of your data,
- the right to rectification (correction) of your data,
- the right to withdraw your consent at any time, without affecting the lawfulness of processing based on the consent given before its withdrawal,
- the right to the erasure of your personal data if the data processing is not necessary for the compliance with a legal obligation or for the exercise of official authority,
- the right to restriction of processing,
- the right to object to processing of personal data,
- the right to data portability
8. You have the right to file a complaint with the President of the Personal Data Protection Office, to the address of the Personal Data Protection Office (ul. Stawki 2, 00-193, Warsaw; www.uodo.gov.pl) if you believe that your personal data are being processed contrary to the law.
9. Providing your personal data is voluntary. The consequences of not providing the data referred to in in paragraph 3(c) is that you will receive standard (not vegetarian) meal during the dinner. Not providing the data referred to in paragraph 3 (b) has no consequences. If you do not agree to processing your image and voice sound via Zoom instant messaging, you should mute your microphone and turn off your camera.
