Pursuant to art. 13 (1) and (2) of Regulation of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation) (Official Journal of the European Union L 119 of 4 May 2016) the IFJ PAN informs you that:
1. The controller of your personal data is the Henryk Niewodniczański Institute of Nuclear Physics Polish Academy of Sciences (IFJ PAN), ul. Radzikowskiego 152, 31-342 Kraków, represented by the Director of IFJ PAN.
2. The IFJ PAN appointed the Personal Data Protection Officer. You can contact the Data Protection Officer by e-mail at: dpo@ifj.edu.pl
3. Your personal data will be processed pursuant to:
- art. 6, item. 1 letter b of the General Data Protection Regulation of 27 April 2016 (processing necessary for the execution of the workshop participation contract),
- art. 6, item. 1 letter c of the General Data Protection Regulation of 27 April 2016 in connection with the provisions of the Accounting Act of 29 September 1994 for the purpose of book-keeping and tax documentation managing,
- art. 6 item 1, letter a of the General Data Protection Regulation of 27 April 2016 (if the consent has been given) for the purpose of publishing the list of participants on the workshop website and preparing meals during the workshop dinner in accordance with your food preferences.
4. The recipients of your personal data will be solely:
- entities cooperating with IFJ PAN within the framework of scientific collaborations or providing services for IFJ PAN (courier, postal, publishing and internet services for IFJ PAN)
- entities and institutions authorized to access or receive personal data on the basis of legal provisions (e.g. court, police, tax authorities),
- all persons having access to the Internet and to the lists of workshop participants published there, if your consent has been given to the processing of personal data in order to publish the list of workshop participants at the indico.ifj.edu.pl platform.
- zoom instant messaging providers.
5. Your personal data will be retained
- for the proper organization and settlement of the workshop based on the provisions of the law on archiving documents and in accordance with the Uniform List of Files in force at IFJ PAN - for a period of 5 years, counting from January 1 of the year following the final settlement of the case
- for the purpose of informing about future scientific events organized by IFJ PAN - until the consent for processing is withdrawn,
- for the purpose of publishing the participants’ list on the workshop website and in order to prepare proper meals during the workshop - until the workshop is finished.
6. Your personal data will not be subject to automated processing, including profiling.
7. You have the right to:
- demand access to your personal data and receive a copy of your personal data,
- rectification (correction) of your personal data,
- withdraw your consent at any time, without affecting the lawfulness of processing based on the consent given before its withdrawal,
- erasure of your personal data if the data processing is not necessary for the compliance with a legal obligation or for the exercise of official authority,
- restriction of processing of your personal data,
- lodge a complaint to the President of the Office for Personal Data Protection (ul. Stawki 2, 00 - 193 Warszawa)
8. Providing your personal data is voluntary. The consequence of not providing the data referred to in point 3 letter a) and b) will be the inability to participate in the workshop. The consequences of not providing the data referred to in point 3 letter c) will be:
- your name will not be published at the participant list or/and
- you will receive standard (not vegetarian) meal during the dinner.

Zgodnie z art. 13 ust. 1 i ust. 2 rozporządzenia Parlamentu Europejskiego i Rady (UE) 2016/679 z 27 kwietnia 2016 r. w sprawie ochrony osób fizycznych w związku z przetwarzaniem danych osobowych
i w sprawie swobodnego przepływu takich danych oraz uchylenia dyrektywy 95/46/WE (Dz. Urz. UE L 119 z 04.05.2016, str. 1, z późn. zm.) (dalej: RODO) informujemy, że:
1. administratorem Pani/Pana danych osobowych jest Dyrektor Instytutu Fizyki Jądrowej im. Henryka Niewodniczańskiego Polskiej Akademii Nauk, ul. Radzikowskiego 152, 31-342 Kraków,
2. kontakt z Inspektorem Ochrony Danych Osobowych możliwy jest pod adresem dpo@ifj.edu.pl,
3. Pani/Pana dane osobowe przetwarzane będą na podstawie:
- art. 6 ust. 1 lit. b RODO (przetwarzanie niezbędne do wykonania umowy uczestnictwa w warsztatach),
- art. 6 ust. 1 lit. c RODO, w związku z przepisami ustawy z dnia 29 września 1994 r. o rachunkowości w celu związanym z prowadzeniem ksiąg rachunkowych i dokumentacji podatkowej,
- art. 6 ust. 1 lit. a. RODO (w przypadku wyrażenia zgody na opublikowanie listy uczestników na stronie internetowej warsztatów oraz w przypadku wyrażenia zgody na udział w kolacji konferencyjnej i związane z tym przetwarzanie danych dot. preferencji żywieniowych).
Następnie Pani/Pana dane będą przechowywane w celu wypełnienia obowiązku archiwizacji dokumentów wynikającego z ustawy z dnia 14 lipca 1983 r. o narodowym zasobie archiwalnym i archiwach.
4. Pani/Pana dane będą udostępniane następującym odbiorcom lub kategoriom odbiorców:
- podmiotom współpracującym z IFJ PAN w ramach współpracy naukowej oraz dostawcom niezbędnych usług na podstawie stosownych umów ( usługi kurierskie, pocztowe, wydawnicze oraz internetowe dla IFJ PAN),
- podmiotom i instytucjom upoważnionym do dostępu lub otrzymania danych osobowych na podstawie przepisów prawa ( np. sąd, policja, organy podatkowe),
- wszystkim osobom mającym dostęp do Internetu i opublikowanych tam list uczestników konferencji w przypadku wyrażenia przez Panią/Pana zgody na przetwarzanie danych osobowych w celu publikacji listy uczestników konferencji na platformie indico.ifj.edu.pl.
- dostawcy komunikatora internetowego zoom.
5. Pani/Pana dane będą przechowywane:
- w celu prawidłowego zrealizowania i rozliczenia warsztatów na podstawie przepisów prawa o archiwizacji dokumentów oraz zgodnie z obowiązującym w IFJ PAN Jednolitym Rzeczowym Wykazem Akt – przez okres 5 lat, licząc od 1 stycznia roku następnego po ostatecznym załatwieniu sprawy,
- w celu publikacji listy uczestników na stronie internetowej warsztatów oraz w celu przygotowania właściwych posiłków w czasie warszatatów - do zakończenia warsztatów.
6. Pani/Pana dane osobowe nie będą podlegały zautomatyzowanemu przetwarzaniu, w tym profilowaniu,
7. Posiada Pani/Pan prawo do:
- dostępu do swoich danych oraz otrzymania ich kopii,
- do sprostowania (poprawiania) swoich danych,
- do cofnięcia zgody w dowolnym momencie bez wpływu na zgodność z prawem przetwarzania, którego dokonano na podstawie zgody przed jej cofnięciem,
- do usunięcia danych osobowych (nie dotyczy sytuacji, gdy przetwarzanie danych jest niezbędne do wywiązania się z obowiązku wynikającego z przepisu prawa),
- do ograniczenia przetwarzania danych,
- do wniesienia skargi do Prezes UODO (na adres Urzędu Ochrony Danych Osobowych, ul. Stawki 2, 00 - 193 Warszawa),
8. Podanie danych osobowych jest dobrowolne. Konsekwencją niepodania danych, o których mowa w pkt 3 lit. a) i b) będzie brak możliwości wzięcia udziału w warsztatach. Konsekwencjami niepodania danych, o których mowa w pkt. 3 lit. c) będą: - Twoje imię i nazwisko nie zostanie opublikowane na liście uczestników i/lub - podczas kolacji otrzymasz standardowy (nie wegetariański) posiłek.
