Informacja o przetwarzaniu danych osobowych – KOPAL WORKSHOP

Zgodnie z art. 13 ust. 1 i ust. 2 rozporządzenia Parlamentu Europejskiego i Rady (UE) 2016/679 z 27 kwietnia 2016 r. w sprawie ochrony osób fizycznych w związku z przetwarzaniem danych osobowych i w sprawie swobodnego przepływu takich danych oraz uchylenia dyrektywy 95/46/WE (Dz. Urz. UE L 119 z 04.05.2016, str. 1, z późn. zm.) (dalej: RODO) informujemy, że:

Administratorem Pani/Pana danych osobowych jest  Instytut Fizyki Jądrowej im. Henryka Niewodniczańskiego Polskiej Akademii Nauk (IFJ PAN), ul. Radzikowskiego 152, 31-342 Kraków;

Kontakt z Inspektorem Ochrony Danych Osobowych możliwy jest pod adresem dpo@ifj.edu.pl;

Pani/Pana dane osobowe mogą być przetwarzane w celu i na podstawie:

• w celu zawarcia oraz wykonania umowy uczestnictwa w wydarzeniu pn. KOPAL Workshop (dalej: Wydarzenie), na podstawie art. 6 ust. 1 lit. b RODO;
• w celu organizacji Wydarzenia, na podst. art. 6 ust. 1 lit. e RODO, w zw. z przepisami Ustawy z dnia 10 kwietnia 2010 r. o Polskiej Akademii Nauk, §6 i §7 Statutu IFJ PAN oraz realizowaną na tej podstawie misją IFJ PAN, jaką jest tworzenie wiedzy niezbędnej do rozwiązywania problemów i wyzwań stojących przed współczesną nauką, społeczeństwem oraz gospodarką, a także komunikowanie tej wiedzy;
• w celu udostępnienia listy uczestników na stronie internetowej Wydarzenia: www.indico.ifj.edu.pl (na podst. art. 6 ust. 1 lit. a RODO  - tj. na podstawie udzielonej zgody);
• w celu wydania przepustki umożliwiającej wejście na teren IFJ PAN, oraz dane z monitoringu wizyjnego, którym objęty jest teren IFJ PAN,  będą przetwarzane na podst. art. 6 ust 1 lit. e RODO oraz art. 6 ust. 1 lit. c RODO,  w związku z przepisami Ustawy z dnia 30 kwietnia 2010 r. o Polskiej Akademii Nauk, ustawy  z dnia 29 listopada 2000 r. Prawo atomowe oraz ustawy z dnia 22 sierpnia 1997 r. o ochronie osób i mienia, zgodnie z Regulaminem wstępu na teren IFJ PAN, w celu zapewnienia bezpieczeństwa na terenie Instytutu, ochrony tajemnicy IFJ PAN oraz ochrony osób  i mienia;
• w celach dokumentacyjnych, informacyjnych i promowania nauki, poprzez dokumentowanie Wydarzenia w postaci zdjęć (na podst. art. 6 ust. 1 lit. a RODO tj. na podstawie udzielonej zgody na utrwalanie wizerunku) oraz upowszechnianie wiedzy w zakresie działalności Instytutu (na podstawie art. 6 ust. 1 lit. e RODO, w zw. z art. 81 ust. 1 ustawy z dnia 4 lutego 1994 r. o prawie autorskim i prawach pokrewnych – tj. na podstawie udzielonego zezwolenia na rozpowszechnianie wizerunku) na stronie internetowej IFJ PAN i Laboratoire de Physique des 2 Infinis Irène Joliot-Curie, w Biuletynie IFJ PAN, na oficjalnych kontach w mediach społecznościowych (Facebook) IFJ PAN;
• w celu wywiązania się z obowiązków prawnych w zakresie dokumentacji finansowo-księgowej i sprawozdawczej (na podst. art. 6 ust. 1 lit. c RODO, w oparciu o przepisy prawa podatkowego i ustawy o rachunkowości);
• w celu dochodzenia roszczeń lub obrony przed roszczeniami, jeśli w przyszłości zaistniałaby taka konieczność, w związku z zawartą umową (na podst. art. 6 ust. 1 lit. f RODO, tj. uzasadnionego interesu administratora).

W zakresie rozpowszechniania wizerunku, posiada Pani/Pan prawo do wniesienia sprzeciwu na jego rozpowszechnianie.  Sprzeciw można wnieść:

• przesyłając wiadomość pocztą elektroniczną na adres dyrektor@ifj.edu.pl,
• przesyłając wiadomość pocztą tradycyjną na adres Administratora,
• osobiście stawiając się w siedzibie Administratora.

Pani/Pana dane będą udostępniane następującym odbiorcom lub kategoriom odbiorców:

• podmiotom i instytucjom upoważnionym do dostępu lub otrzymania danych osobowych na podstawie przepisów prawa (np. sąd, policja, organy podatkowe);
• wszystkim osobom mającym dostęp do Internetu i opublikowanych tam list uczestników i zdjęć.

Dane osobowe zarejestrowane przez system monitoringu wizyjnego nie będą udostępniane podmiotom trzecim. Mogą zostać udostępnione wyłącznie organom upoważnionym na podstawie odrębnych przepisów prawa.

Pani/Pana dane będą przechowywane:

• celu prawidłowego zrealizowania wydarzenia na podstawie przepisów prawa o archiwizacji dokumentów – przez okres 5 lat, licząc od 1 stycznia roku następnego po ostatecznym załatwieniu sprawy;
• w celu publikacji listy uczestników na stronie internetowej – do zakończenia wydarzenia lub do momentu cofnięcia zgody na przetwarzanie;
• dane osobowe w postaci wizerunku będą przechowywane do momentu cofnięcia zezwolenia na jego rozpowszechnianie lub do momentu wniesienia sprzeciw na jego rozpowszechnianie;
• dane osobowe będą zarejestrowane w formie elektronicznej w postaci obrazu z monitoringu wizyjnego oraz przechowywane przez okres nie dłuższy niż 90 dni.  W przypadku, w którym nagrania obrazu stanowią dowód w postępowaniu prowadzonym na podstawie prawa lub administrator powziął wiadomość, iż mogą one stanowić dowód w postępowaniu, termin przetwarzania ulega przedłużeniu do czasu prawomocnego zakończenia postępowania;
• dane osobowe podane w celu wydania przepustki przechowywane będą – na podstawie przepisów prawa o archiwizacji dokumentów oraz zgodnie z obowiązującym w IFJ PAN Jednolitym Rzeczowym Wykazem Akt – przez okres 10 lat, licząc od 1 stycznia roku następnego po ostatecznym załatwieniu sprawy;
• dane osobowe, o których mowa w pkt. 3 lit. g) przetwarzane będą, jeśli zajdzie taka potrzeba, przez okres niezbędny do dochodzenia roszczeń lub obrony przed roszczeniami.

Pani/Pana dane osobowe nie będą podlegały zautomatyzowanemu przetwarzaniu, w tym profilowaniu.

Posiada Pani/Pan prawo do żądania realizacji swoich praw wynikających z RODO, tj.:

• dostępu do swoich danych oraz otrzymania ich kopii;
• sprostowania (poprawiania) swoich danych (w przypadku monitoringu wizyjnego w ograniczonym zakresie);
• usunięcia danych osobowych (nie dotyczy sytuacji, gdy przetwarzanie danych jest niezbędne do wywiązania się z obowiązku wynikającego z przepisu prawa);
• ograniczenia przetwarzania danych;
•  wniesienia sprzeciwu wobec przetwarzania.

Ma Pani/Pan prawo do wniesienia skargi do Prezesa UODO (na adres Urzędu Ochrony Danych Osobowych, ul. Stanisława Moniuszki 1A, 00-014 Warszawa; www.uodo.gov.pl), jeśli uważa Pani/Pan, że dane osobowe są przetwarzane niezgodnie z wymogami prawnymi.

Podanie danych osobowych, o których mowa w pkt. 3 lit. a), b), d), f) i g) jest niezbędne do wzięcia udziału w Wydarzeniu. Konsekwencją niepodania danych osobowych, będzie brak możliwości wzięcia udziału w Wydarzeniu. Podanie danych, o których mowa w pkt. 3 c) i e) jest dobrowolne, a ich niepodanie nie niesie ze sobą konsekwencji.

Information on the Processing of Personal Data – KOPAL WORKSHOP

Pursuant to Article 13(1) and Article 13(2) of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (Official Journal of the EU L 119 of 04.05.2016, p. 1, as amended) (hereinafter: GDPR), we hereby inform you that:

The Controller of your personal data is the Henryk Niewodniczański Institute of Nuclear Physics of the Polish Academy of Sciences (IFJ PAN), ul. Radzikowskiego 152, 31-342 Kraków;

You may contact the Data Protection Officer at: dpo@ifj.edu.pl;

Your personal data may be processed for the following purposes and on the following legal bases:

• for the purpose of concluding and performing the participation agreement for the event entitled KOPAL Workshop (hereinafter: the Event), pursuant to Article 6(1)(b) of the GDPR;

• for the purpose of organizing the Event, pursuant to Article 6(1)(e) of the GDPR, in conjunction with the provisions of the Act of 30 April 2010 on the Polish Academy of Sciences, §6 and §7 of the Statute of IFJ PAN, and the mission of IFJ PAN carried out on this basis, which is to create knowledge necessary to address problems and challenges faced by contemporary science, society and the economy, as well as to communicate this knowledge;

• for the purpose of publishing the list of participants on the Event website: www.indico.ifj.edu.pl (pursuant to Article 6(1)(a) of the GDPR – i.e. based on your consent);

• for the purpose of issuing an access pass enabling entry to the premises of IFJ PAN, and the data from the video surveillance system covering the premises of IFJ PAN will be processed pursuant to Article 6(1)(e) and Article 6(1)(c) of the GDPR, in connection with the provisions of the Act of 30 April 2010 on the Polish Academy of Sciences, the Act of 29 November 2000 – Atomic Law, and the Act of 22 August 1997 on the Protection of Persons and Property, in accordance with the Regulations for Access to the IFJ PAN premises, in order to ensure safety on the premises of the Institute, protect IFJ PAN confidentiality, and protect persons and property;

• for documentation, information and science promotion purposes, by documenting the Event in the form of photographs (pursuant to Article 6(1)(a) of the GDPR, i.e. based on consent to record the image) and disseminating knowledge about the activities of the Institute (pursuant to Article 6(1)(e) of the GDPR, in conjunction with Article 81(1) of the Act of 4 February 1994 on Copyright and Related Rights – i.e. based on consent to disseminate the image) on the IFJ PAN website and the website of Laboratoire de Physique des 2 Infinis Irène Joliot-Curie, in the IFJ PAN Bulletin, and on the official IFJ PAN social media accounts (Facebook);

• for the purpose of fulfilling legal obligations regarding financial, accounting and reporting documentation (pursuant to Article 6(1)(c) of the GDPR, based on tax regulations and the Accounting Act);

• for the purpose of pursuing claims or defending against claims, if such a necessity arises in the future, in connection with the concluded agreement (pursuant to Article 6(1)(f) of the GDPR, i.e. the legitimate interest of the Controller).

With regard to the dissemination of your image, you have the right to object to its dissemination. The objection may be submitted:

• by sending an e-mail to: dyrektor@ifj.edu.pl,

• by sending a traditional letter to the Controller’s address,

• by appearing in person at the Controller’s office.

Your personal data will be disclosed to the following recipients or categories of recipients:

• entities and institutions authorized to access or receive personal data pursuant to legal provisions (e.g. courts, police, tax authorities);

• all persons with access to the Internet and published participant lists and photographs.

Personal data recorded by the video surveillance system will not be disclosed to third parties. They may only be made available to authorities authorized under separate legal provisions.

Your personal data will be stored:

• for the purpose of proper implementation of the Event, pursuant to legal provisions on document archiving – for a period of 5 years, starting from 1 January of the year following the final completion of the matter;

• for the purpose of publishing the participant list on the website – until the end of the Event or until consent to data processing is withdrawn;

• personal data in the form of an image will be stored until consent for its dissemination is withdrawn or until an objection to its dissemination is submitted;

• personal data recorded electronically in the form of video surveillance footage will be stored for a period not exceeding 90 days. If the recordings constitute evidence in proceedings conducted pursuant to law, or the Controller becomes aware that they may constitute such evidence, the processing period shall be extended until the final conclusion of the proceedings;

• personal data provided for the purpose of issuing an access pass will be stored – pursuant to legal provisions on document archiving and in accordance with the applicable Uniform Records Classification System at IFJ PAN – for a period of 10 years, starting from 1 January of the year following the final completion of the matter;

• personal data referred to in point 3(g) will be processed, if necessary, for the period required to pursue claims or defend against claims.

Your personal data will not be subject to automated decision-making, including profiling.

You have the right to request the exercise of your rights under the GDPR, including:

• access to your data and obtaining a copy thereof;

• rectification (correction) of your data (in the case of video surveillance, to a limited extent);

• deletion of personal data (except where processing is necessary to fulfil a legal obligation);

• restriction of processing;

• objection to processing.

You have the right to lodge a complaint with the President of the Personal Data Protection Office (UODO) (at the address: Office for Personal Data Protection, ul. Stanisława Moniuszki 1A, 00-014 Warsaw; www.uodo.gov.pl), if you believe that your personal data is being processed in violation of legal requirements.

Providing the personal data referred to in point 3(a), (b), (d), (f) and (g) is necessary to participate in the Event. Failure to provide such data will result in the inability to participate in the Event. Providing the data referred to in point 3(c) and (e) is voluntary and failure to provide it will not result in any consequences.
