Zgodnie z art. 13 ust. 1 i ust. 2 rozporządzenia Parlamentu Europejskiego i Rady (UE) 2016/679 z 27 kwietnia 2016 r. w sprawie ochrony osób fizycznych w związku z przetwarzaniem danych osobowych
i w sprawie swobodnego przepływu takich danych oraz uchylenia dyrektywy 95/46/WE (Dz. Urz. UE L 119 z 04.05.2016, str. 1, z późn. zm.) (dalej: RODO) informujemy, że:
1. administratorem Pani/Pana danych osobowych jest Instytut Fizyki Jądrowej im. Henryka Niewodniczańskiego Polskiej Akademii Nauk, ul. Radzikowskiego 152, 31-342 Kraków,
2. kontakt z Inspektorem Ochrony Danych Osobowych możliwy jest pod adresem dpo@ifj.edu.pl,
3. Pani/Pana dane osobowe przetwarzane będą na podstawie:
a) art. 6 ust. 1 lit. b RODO – w celu realizacji umowy dotyczącej uczestnictwa w warsztatach;
b) art. 6 ust. 1 lit. f RODO – w celu realizacji prawnie uzasadnionych interesów administratora, zgodnie z Regulaminem wstępu na teren IFJ PAN, w szczególności dla zapewnienia bezpieczeństwa (dotyczy danych pozyskanych w celu wydania przepustek i danych z monitoringu wizyjnego);
c) art. 6 ust. 1 lit. a RODO – (jeśli wyrażono zgodę) w celu informowania o przyszłych wydarzeniach naukowych organizowanych przez IFJ PAN, publikacji listy uczestników na stronie konferencji oraz utrwalenia i publikacji wizerunku uczestnika w materiałach fotograficznych na stronie konferencji;
d) art. 6 ust. 1 lit. a oraz/lub art. 9 ust. 2 lit. a RODO – (jeśli wyrażono zgodę) w celu przygotowania odpowiednich posiłków z uwzględnieniem alergii i preferencji żywieniowych.
4. Ma Pani/Pan prawo w dowolnym momencie wycofać zgodę, bez wpływu na zgodność z prawem przetwarzania, którego dokonano na podstawie zgody przed jej cofnięciem. Cofnięcie zgody może nastąpić poprzez wysłanie wiadomości e-mail na adres: dai_konf@ifj.edu.pl,
5. Pani/Pana dane będą udostępniane następującym odbiorcom lub kategoriom odbiorców:
- podmiotom uprawnionym do uzyskania danych na podstawie przepisów prawa (np. policja, sąd, organy podatkowe)
- podmiotom współpracującym z IFJ PAN w ramach współpracy naukowej lub świadczącym usługi dla IFJ PAN (np. firmy kurierskie, pocztowe),
- każdemu, kto ma dostęp do Internetu – w zakresie publikacji listy uczestników i zdjęć (jeśli wyrażono zgodę).
6. Pani/Pana dane będą przechowywane:
- zgodnie z przepisami prawa dotyczącymi archiwizacji dokumentów – przez okres 5 lat, licząc od 1 stycznia roku następującego po ostatecznym rozliczeniu wydarzenia (dane niezbędne do organizacji i rozliczenia warsztatów), lub przez okres przedawnienia roszczeń przewidziany przepisami prawa,
- w celu informowania o przyszłych wydarzeniach naukowych oraz publikacji zdjęć z warsztatów – przez 2 lata lub do momentu cofnięcia zgody,
- w celu publikacji listy uczestników na stronie warsztatów oraz przygotowania posiłków – do zakończenia warsztatów
- dane z monitoringu wizyjnego – nie dłużej niż 90 dni, a w przypadku, gdy nagranie stanowi dowód w postępowaniu – do jego prawomocnego zakończenia. Dane osobowe udostępnione w celu umożliwienia wejścia na teren IFJ PAN będą przechowywane zgodnie z przepisami o archiwizacji oraz obowiązującym w IFJ PAN Jednolitym Rzeczowym Wykazem Akt – przez okres 10 lat, licząc od 1 stycznia roku następującego po zakończeniu sprawy.
7. Pani/Pana dane osobowe nie będą podlegały zautomatyzowanemu przetwarzaniu, w tym profilowaniu,
8. Przysługują Pani/Panu prawa wynikające z RODO:
- prawo dostępu do danych osobowych i otrzymania ich kopii,
- prawo do sprostowania (poprawienia) danych,
- prawo do usunięcia danych, jeśli przetwarzanie nie jest konieczne do wypełnienia obowiązku prawnego lub wykonywania zadań publicznych,
- prawo do ograniczenia przetwarzania,
- prawo do wniesienia sprzeciwu wobec przetwarzania danych.
9. Podanie danych osobowych, o których mowa w pkt 3 lit. a) i b) jest obowiązkowe – ich niepodanie uniemożliwi udział w warsztatach.
Podanie danych, o których mowa w pkt 3 lit. d), jest dobrowolne – ich niepodanie uniemożliwi przygotowanie posiłków zgodnie z preferencjami żywieniowymi. Podanie danych osobowych, o których mowa w pkt. 3 lit. c) jest również dobrowolne a ich niepodanie nie niesie ze sobą konsekwencji.

Pursuant to art. 13 (1) and (2) of Regulation of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation) (Official Journal of the European Union L 119 of 4 May 2016) the IFJ PAN informs you that:
1. The controller of your personal data is the Henryk Niewodniczański Institute of Nuclear Physics Polish Academy of Sciences (IFJ PAN), ul. Radzikowskiego 152, 31-342 Kraków.
2. The IFJ PAN appointed the Personal Data Protection Officer. You can contact the Data Protection Officer by e-mail at: dpo@ifj.edu.pl
3. Your personal data will be processed pursuant to:
a) art. 6(1) letter b of the GDPR (processing necessary for the execution of the workshop participation contract);
b) art. 6(1) letter f of the GDPR (processing is necessary for the purposes of the legitimate interests pursued by the controller) in accordance with the Regulations for admission to IFJ PAN premises, in order to ensure security - concerns personal data obtained for the purpose of issuing entry-passes and data from video monitoring;
c) art. 6(1) letter a of the GDPR (if the consent has been given) for the purpose of informing about future scientific events organized by IFJ PAN, publishing the participants' list on the conference website, and recording my image in the conference photos as well publishing it on the conference website;
d) art. 6(1) letter a and/or art. 9(2) letter a of the GDPR (if the consent has been given) for the purpose of preparing proper meals with regard to allergies and food preferences.
4. You have the right to withdraw your consent at any time, without affecting the lawfulness of the processing based on the consent given before its withdrawal. Consent may be withdrawn: by sending an email to dpo@ifj.edu.pl,
5. Your personal data may be disclosed to the following recipients or categories of recipients:
• Entities entitled to obtain personal data under provisions of law (e.g. the police, court, tax authorities);
• Entities cooperating with IFJ PAN within the framework of scientific collaborations or providing services for IFJ PAN (e.g. courier, postal services);
• Anyone who has access to the Internet and the lists of conference participants and photos published there (if the consent has been given).
6. Your personal data will be retained:
• Under the provisions of the law on archiving documents for a period of 5 years, counting from 1 January of the year following the final settlement of the case (data necessary for the purpose of the organization and accounting of the conference), or for the period of limitation of claims specified in the provisions of law;
• For the purpose of informing about future scientific events organized by IFJ PAN and for the purpose of publishing conference photos – for two years or until the consent for processing is withdrawn;
• For the purpose of publishing the participants' list on the conference website and for the purpose of preparing proper meals with regard to allergies and food preferences – until the conference is finished;
• personal data from video monitoring will be stored not longer than 90 days, and if the recording is or may be evidence in proceedings conducted on the basis of law - until the final conclusion of the proceedings. Personal data provided to enable entry to the premises of IFJ PAN will be stored on the basis of provisions of law on archiving documents and in accordance with the Uniform Material List of Files in force at IFJ PAN - for a period of 10 years, counting from 1 January of the year following the final settlement of the matter.
7. Your personal data will not be subject to automated processing, including profiling.
8. You have the right to demand the exercise of your rights under the GDPR:
• Demand access to your personal data and receive a copy of your personal data;
• Rectification (correction) of your personal data,
• Erasure of your personal data if the data processing is not necessary for the compliance with a legal obligation or for the exercise of official authority,
• Restriction of processing of your personal data,
• Object to processing of personal data.
9. Providing of your personal data referred to in point 3 a) and b) is obligatory and the consequence of not providing the data is inability to participate in the workshop. Providing your personal data referred to in point d) is voluntary and not providing the data shall result in impossibility of preparing meals according to your food preferences. Providing your personal data referred to in point 3 c) is voluntary and not providing the data has no consequences.
