General Data Protection Regulation (GDPR) Information
We ask for the processing of personal data by the Henryk Niewodniczański Institute of Nuclear Physics Polish Academy of Sciences for: · the purpose of publishing the list of participants on the conference websites
· the purpose of publishing conference photos on the conference websites
· the purpose of informing about my food preferences
· the purpose of informing me about future scientific events organized by IFJ PAN
Data protection clause
Pursuant to Article 13 (1) and (2) of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (Official Journal of the EU L 119 of 4 May 2016, p. 1, as amended) (hereinafter: the GDPR), please be advised that:
1. The controller of your personal data is the Institute of Nuclear Physics Polish Academy of Sciences (IFJ PAN), ul. Radzikowskiego 152, 31-342 Kraków. 2. You can contact the Data Protection Officer at dpo@ifj.edu.pl
3. Your personal data will be processed pursuant to:
a) art. 6, item. 1 letter b of the GDPR (processing necessary for the execution of the conference participation contract),
b) art. 6, item. 1 letter c of the GDPR in connection with the provisions of the Accounting Act of 29 September 1994 for the purpose of book-keeping and tax documentation managing,
c) art. 6, item 1 letter f (processing is necessary for the purposes of the legitimate interests pursued by the controller) in accordance with the Regulations for admission to IFJ PAN premises, in order to ensure security - concerns personal data obtained for the purpose of issuing entry-passes and data from video monitoring,
d) art. 6 item 1 letter a) and/or art. 9 item. 2 letter a) of the GDPR (if the consent has been given) for the purpose of preparing proper meals with regard to your food preferences,
e) art. 6 item. 1 letter a of the GDPR (if the consent has been given) for the purpose of informing about future scientific events organized by IFJ PAN, publishing the participants’ list and conference photos on the conference websites relating to the activities of IFJ PAN.
4. You have the right to withdraw your consent at any time, without affecting the lawfulness of the processing based on the consent given before its withdrawal. In order to withdraw your consent, please send an e-mail to eiclhc@ifj.edu.pl
5. Your personal data may be disclosed to the following recipients or categories of recipients:
a) entities that cooperate with IFJ PAN as part of scientific cooperation and providers of necessary services under the relevant agreements (e.g. courier, postal services),
b) entities and institutions entitled to obtain personal data under provisions of law (e.g. the police, court, tax authorities),
c) hotel, in order to make a reservation,
d) anyone who has access to the Internet and the lists of conference participants and photos published there – (if the consent has been given),
e) Zoom instant messaging provider.
6. Your personal data will be retained:
- under the provisions of the law on archiving documents and in accordance with the Uniform Material List of Files in force at IFJ PAN for a period of 5 years, counting from 1 January of the year following the final settlement of the case (data necessary for the purpose of the organization and accounting of the conference), or for the period of limitation of claims specified in the provisions of law,
- personal data from video monitoring will be stored not longer than 90 days, and if the recording is or may be evidence in proceedings conducted on the basis of law - until the final conclusion of the proceedings. Personal data provided to enable entry to the premises of IFJ PAN will be stored on the basis of provisions of law on archiving documents and in accordance with the Uniform Material List of Files in force at IFJ PAN - for a period of 10 years, counting from 1 January of the year following the final settlement of the matter,
- for the purpose of informing about future scientific events organized by IFJ PAN and for the purpose of taking and publishing conference photos – until the consent for processing is withdrawn;
- for the purpose of preparing proper meals with regard to your food preferences – until the conference is finished.
7. Your personal data will not be subject to automated processing, including profiling.
8. You have the right to demand the exercise of your rights under the GDPR:
- access to your personal data and receive a copy of your personal data,
- rectification (correction) of your personal data,
- erasure of your personal data if the data processing is not necessary for the compliance with a legal obligation or for the exercise of official authority,
- restriction of processing of your personal data,
- object to processing of personal data.
9. You have the right to file a complaint with the President of the Personal Data Protection Office (to the address of the Personal Data Protection Office, ul. Stawki 2, 00-193, Warsaw; www.uodo.gov.pl) if you believe that your personal data are being processed contrary to the law.
10. Providing of your personal data referred to in point 3 a), b) and c) is obligatory and the consequence of not providing the data is inability to participate in the conference. Providing your personal data referred to in point d) is voluntary and not providing the data shall result in impossibility of preparing meals according to your food preferences. Providing your personal data referred to in point e) is voluntary and not providing the data has no consequences.
11. If you do not agree to processing your image and voice sound via Zoom instant messaging, you should mute your microphone and turn off your camera.
