Pursuant to Art. 13 of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation , hereinafter referred to as "GDPR"), IFJ PAN informs that:

1. The joint controllers of your personal data are: National Centre for Nuclear Research, represented by the Director of NCBJ and the Henryk Niewodniczański Institute of Nuclear Physics, Polish Academy of Sciences (hereinafter referred to as “IFJ PAN”) represented by the Director of IFJ PAN.
2. NCBJ has appointed a Personal Data Protection Inspector. It is possible to contact the Inspector by e-mail: iod@ncbj.gov.pl or by phone at +48-22-273-22-31.
3. IFJ PAN has appointed a Personal Data Protection Inspector. It is possible to contact the Inspector by e-mail: dpo@ifj.edu.pl or by phone +48-12-662-80-80.
4. Your personal data will be processed on the basis of:
5. Article 6 (1) (b) GDPR for the purpose of executing the contract of participation in the Seminar, until the settlement of the Project under which the Seminar is organized, no longer than 5 years
6. personal data obtained for the purpose of issuing passes and data from video monitoring covering the IFJ PAN premises will be processed pursuant to Art. 6(1)(e) of the GDPR, in accordance with the Regulations for admission to the IFJ PAN premises, in order to ensure security; personal data from video monitoring will be stored for a period not longer than 90 days, and if the recording is or may be evidence in proceedings conducted on the basis of law - until the final conclusion of the proceedings. Personal data provided to enable entry to the premises of the IFJ PAN will be stored on the basis of the provisions of the law on archiving documents and in accordance with the Uniform Material List of Files in force at the IFJ PAN - for a period of 10 years, counting from January 1 of the year following the final settlement of the matter.
7. Article 6(1)(c) of the GDPR, in connection with the provisions of the Act of September 29, 1994 on Accounting, for purposes related to keeping accounting books and tax documentation, until the Project is settled and the archiving period expires, no longer than 5 years
8. Article 6(1)(a) of the GDPR - in the event of consent - to inform about future scientific events, until consent is withdrawn
9. Article 6(1)(a) of the GDPR - in the event of consent to the recording and dissemination of the image - for the purpose informing about the event in publications and social media, until consent is withdrawn
10. Article 6(1)(a) of the GDPR - in the event of consent to the recording of the sound of the voice and its dissemination - for the purpose of reporting in publications and social media about the event, until consent is withdrawn
11. Article 6(1)(a) of the GDPR - in the event of consent to participate in a confernce lunch and the related processing of data regarding dietary preferences, until consent is withdrawn
12. Article 9(2)(a) of the GDPR - in the event of consent to participate in a conference lunch and the related processing of data regarding dietary preferences, until the consent is withdrawn.

Then, your data will be stored in order to fulfill the obligation to archive documents resulting from the Act of July 14, 1983 on the national archival resources and archives and in accordance with the Uniform List of Files in force at IFJ PAN and NCBJ.

1. Your data will be made available to the following recipients or categories of recipients:

• entities cooperating with IFJ PAN or NCBJ as part of scientific cooperation,
• suppliers of necessary services based on appropriate contracts (organizational and technical support for Events, legal, courier, postal, publishing and Internet services),
• Zoom instant messaging provider,
• entities and institutions authorized to access or receive personal data on the basis of legal provisions (e.g. court, police, tax authorities),
• to the Ministry of Education and Science under the provisions of law and binding contracts for the financing of the project "Big Science for Polish Innovation", conducted by the National Centre for Nuclear Research - due to the need for accountability of the funded project,
• to all those who have access to the Internet and photos/recordings published there.

1. Your personal data will not be subject to automated processing, including profiling,
2. You have the right to request the exercise of your rights under the GDPR, i.e.:

• access to your data and receive a copy of it,
• rectification (correction) of your data,
• withdraw consent at any time without affecting the lawfulness of processing based on consent before its withdrawal,
• deletion of personal data (does not apply if the processing of data is necessary to comply with an obligation under the law),
• restrictions on data processing,
• to object to the processing,
• for data transfer.

1. You have the right to lodge a complaint with the President of the Personal Data Protection Office (to the address of the Personal Data Protection Office, Stawki 2, 00 - 193 Warsaw); www.uodo.gov.pl), if you believe that your personal data is being processed in violation of legal requirements.
2. Providing your personal data is voluntary. The consequence of failure to provide the data referred to in point 3 a), b) and c) will be the inability to participate in the event. Failure to provide the data referred to in point 3 d), e), and f) has no consequences. Failure to provide the data referred to in point 3 g) and h) will result in the inability to prepare a meal in accordance with your food preferences. Withdrawal of consent for the processing of personal data can be sent by e-mail to the following address: iod@ncbj.gov.pl or dpo@ifj.edu.pl, by traditional mail to the address NCBJ, Andrzeja Sołtana 7, 05-400 Otwock or IFJ PAN, Radzikowskiego 152, 31-342 Kraków.

Zgodnie z art. 13 Rozporządzenia Parlamentu Europejskiego i Rady (UE) 2016/679 z dnia 27 kwietnia 2016 r. w sprawie ochrony osób fizycznych w związku z przetwarzaniem danych osobowych i w sprawie swobodnego przepływu takich danych oraz uchylenia dyrektywy 95/46/WE (ogólne rozporządzenie o ochronie danych, dalej „RODO”) IFJ PAN informuje, że: 

1. Współadministratorami Pani/Pana danych osobowych są: Narodowe Centrum Badań Jądrowych, reprezentowane przez Dyrektora NCBJ i Instytut Fizyki Jądrowej im. Henryka Niewodniczańskiego PAN reprezentowany przez Dyrektora IFJ PAN. 
2. NCBJ wyznaczył Inspektora Ochrony Danych Osobowych. Z Inspektorem możliwy jest kontakt przez e-mail: iod@ncbj.gov.pl lub pod nr telefonu +48-22-273-22-31.

4. Pani/Pana dane osobowe będą przetwarzane na podstawie:
5. art. 6 ust. 1 lit. b RODO w celu wykonania umowy uczestnictwa w Seminarium, do czasu rozliczenia Projektu, w ramach którego organizowane jest Seminarium, nie dłużej niż 5 lat
6. dane osobowe pozyskane w celu wydania przepustek oraz dane z monitoringu wizyjnego, którym objęty jest teren IFJ PAN, przetwarzane będą na podstawie art. 6 ust 1 lit. e RODO, zgodnie z Regulaminem wstępu na teren IFJ PAN, w celu zapewnienia bezpieczeństwa; dane osobowe pochodzące z monitoringu wizyjnego  będą przechowywane przez okres nie dłuższy niż 90 dni, a w przypadku gdy nagranie jest lub może być dowodem w postępowaniu prowadzonym na podstawie prawa – do czasu prawomocnego zakończenia postępowania. Dane osobowe podane w celu umożliwienia wstępu na teren IFJ PAN przechowywane będą na podstawie przepisów prawa o archiwizacji dokumentów oraz zgodnie z obowiązującym w IFJ PAN Jednolitym Rzeczowym Wykazem Akt – przez okres 10 lat, licząc od 1 stycznia roku następnego po ostatecznym załatwieniu sprawy
7. art. 6 ust. 1 lit. c RODO, w związku z przepisami ustawy z dnia 29 września 1994 r. o rachunkowości w celu związanym z prowadzeniem ksiąg rachunkowych i dokumentacji podatkowej, do czasu rozliczenia Projektu i upływu okresu archiwizacji, nie dłużej niż 5 lat
8. art. 6 ust. 1 lit. a RODO  - w przypadku wyrażenia zgody - w celu informowania o przyszłych wydarzeniach naukowych, do czasu cofnięcia zgody
9. art. 6 ust. 1 lit. a RODO  - w przypadku wyrażenia zgody na utrwalanie i rozpowszechnianie wizerunku – w celu informowania w publikacjach i social mediach o wydarzeniu, do czasu cofnięcia zgody
10. art. 6 ust. 1 lit. a RODO  - w przypadku wyrażenia zgody na nagranie brzmienia głosu i jego rozpowszechnianie - w celu informowania w publikacjach i social mediach o wydarzeniu, do czasu cofnięcia zgody
11. art. 6 ust. 1 lit. a. RODO  - w przypadku wyrażenia zgody na udział w lunchu konferencyjnym i związane z tym przetwarzanie danych dot. preferencji żywieniowych, do czasu cofnięcia zgody
12. art. 9 ust. 2 lit. a. RODO  - w przypadku wyrażenia zgody na udział w lunchu konferencyjnym i związane z tym przetwarzanie danych dot. preferencji żywieniowych, do czasu cofnięcia zgody. 

Następnie Pani/Pana dane będą przechowywane w celu wypełnienia obowiązku archiwizacji dokumentów wynikającego z ustawy z dnia 14 lipca 1983 r. o narodowym zasobie archiwalnym 
i archiwach oraz zgodnie z obowiązującym w IFJ PAN i NCBJ Jednolitym Rzeczowym Wykazem Akt.

1. Pani/Pana dane będą udostępniane następującym odbiorcom lub kategoriom odbiorców:

• podmiotom współpracującym z IFJ PAN lub NCBJ w ramach współpracy naukowej,
• dostawcom niezbędnych usług na podstawie stosownych umów (obsługa organizacyjna i techniczna Wydarzenia, usługi prawne, kurierskie, pocztowe, wydawnicze oraz internetowe),
• dostawcy komunikatora internetowego Zoom,
• podmiotom i instytucjom upoważnionym do dostępu lub otrzymania danych osobowych na podstawie przepisów prawa ( np. sąd, policja, organy podatkowe),
• Ministerstwu Edukacji i Nauki na podstawie przepisów prawa i wiążących umów na finansowanie projektu „Wielka Nauka dla Polskiej Innowacyjności”, prowadzonego przez Narodowe Centrum Badań Jądrowych – w związku z koniecznością rozliczalności finansowanego projektu,
• wszystkim osobom mającym dostęp do Internetu i opublikowanych tam zdjęć/nagrań.

1. Pani/Pana dane osobowe nie będą podlegały zautomatyzowanemu przetwarzaniu, w tym profilowaniu,
2. Posiada Pani/Pan prawo do żądania realizacji swoich praw wynikających z RODO, tj.:

• dostępu do swoich danych oraz otrzymania ich kopii,
• sprostowania (poprawiania) swoich danych, 
• cofnięcia zgody w dowolnym momencie bez wpływu na zgodność z prawem przetwarzania, którego dokonano na podstawie zgody przed jej cofnięciem,
• usunięcia danych osobowych (nie dotyczy sytuacji, gdy przetwarzanie danych jest niezbędne do wywiązania się z obowiązku wynikającego z przepisu prawa),
• ograniczenia przetwarzania danych,
• do wniesienia sprzeciwu wobec przetwarzania,
• do przenoszenia danych.

1. Ma Pani/Pan prawo do wniesienia skargi do Prezesa UODO (na adres Urzędu Ochrony Danych Osobowych, ul. Stawki 2, 00 - 193 Warszawa; www.uodo.gov.pl), jeśli uważa Pani/Pan, że dane osobowe są przetwarzane niezgodnie z wymogami prawnymi.
2. Podanie przez Panią/Pana danych osobowych jest dobrowolne. Konsekwencją niepodania danych, o których mowa w pkt. 3 lit. a), lit. b) i lit. c) będzie brak możliwości wzięcia udziału w wydarzeniu. Niepodanie danych, o których mowa w pkt. 3 lit. d), lit. e), i lit. f)  nie niesie ze sobą konsekwencji. Niepodanie danych, o których mowa w pkt. 3 lit. g) i h) skutkować będzie niemożliwością przygotowania posiłku zgodnego z Pani/Pana preferencjami żywnościowymi. Wycofanie zgody na przetwarzanie danych osobowych można przesłać e-mailem na adres: iod@ncbj.gov.pl lub dpo@ifj.edu.pl, pocztą tradycyjną na adres NCBJ, ul.
